Care Management
Our Care Management Services will give you peace of mind by creating a plan that identifies and solves problems before they occur. Our services can be transitional or longer term depending on the clients need. After a thorough assessment and identification of care goals we find the right services for your loved one.
- Memory Loss
Whether your loved one is mildly confused, or has received an Alzheimer or dementia diagnosis, we can support and help them live their best day. We know that all types of cognitive impairments are progressive and degenerative. Together we will put a plan in place for the changes that inevitably happen. - Aging in Place
We bring the care to you. Our goal is to promote independence and quality of life in a safe environment. This may be care in the home, adult day care, technology, or possible age in place renovations. - Care Coordination
Our services include recommending caregivers, medication management, monitoring your loved ones mental and physical health, and reporting to family members and physicians.
Transitional Housing Placement Services
If you find that living at home is not the best choice, we can help guide you with our Transitional Housing Care Placement Service. We actively facilitate your loved one’s transition to an appropriate community.
- We develop an initial care plan
- We meet with the community team to ensure that the care plan is being followed
- We provide support for any transition behavioral management issues that my arise after the transition
- We are responsive and available for any concerns from you or the community during the 60 day follow up period.
Deborah is a Residential Care Facilities for the Elderly (RCFE) Administrator. Her highly successful, hands-on experience managing memory care and assisted living communities offers you a unique level of expertise resulting in a higher level of care for your loved one.

As a care manager we help you sort through the puzzle and slow down the discharge process so we can figure out the best short and long-term solutions. Better yet, there are ways to ensure that both you and your aging parent are prepared for a medical emergency.
