Often, the first time we realize an aging parent needs help is when we get a call from a neighbor or the hospital. All it takes is one bad fall, or an extended illness and you are scrambling for care. Care options can be confusing and overwhelming. We can help guide you through those next steps. As a care manager we help you sort through the puzzle and slow down the discharge process so we can figure out the best short and long-term solutions.
